KEY POINTS
- Patients with HBe antigen-positive or HBe antigen-negative chronic hepatitis B (previously referred to as immune clearance or immune escape phases) should be considered for antiviral therapy. Treatment is indicated for those with any of the following: high hepatitis B virus (HBV) DNA; elevated alanine aminotransferase levels or evidence of inflammation; fibrosis on liver biopsy or marked fibrosis on FibroScan®.
- All patients with cirrhosis are candidates for treatment.
- Patient and doctor attention to the issue of adherence is critical for the success of therapy.
- Entecavir and tenofovir are first-line treatment options for oral antiviral therapy.
- Pegylated interferon is an alternative option in some patients.
